Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that enables research into pragmatic trials. It collects and shares cleaned trial data and ratings using PRECIS-2, permitting multiple and varied meta-epidemiological research studies to evaluate the effect of treatment on trials that have different levels of pragmatism as well as other design features.
Background
Pragmatic trials are increasingly recognized as providing real-world evidence for clinical decision making. However, the use of the term "pragmatic" is not consistent and its definition and assessment requires clarification. The purpose of pragmatic trials is to guide the practice of clinical medicine and policy decisions rather than verify a physiological hypothesis or clinical hypothesis. A pragmatic trial should also strive to be as close to real-world clinical practice as possible, such as its recruitment of participants, setting and design as well as the execution of the intervention, and the determination and analysis of outcomes as well as primary analysis. This is a significant difference between explanatory trials, as described by Schwartz and Lellouch1 that are designed to prove a hypothesis in a more thorough way.
The trials that are truly pragmatic should be careful not to blind patients or clinicians in order to result in distortions in estimates of treatment effects. Many RCTs that do not meet the criteria for pragmatism, but have features that are contrary to pragmatism have been published in journals of different types and incorrectly labeled pragmatic. This can lead to false claims about pragmatism, and the usage of the term should be made more uniform. The development of the PRECIS-2 tool, which provides an objective and standard assessment of pragmatic characteristics, is a good first step.
Methods
In a practical study the aim is to inform clinical or policy decisions by demonstrating how an intervention can be integrated into routine care in real-world situations. Explanatory trials test hypotheses regarding the causal-effect relationship in idealized environments. In this way, pragmatic trials can have lower internal validity than studies that explain and be more prone to biases in their design analysis, conduct, and design. Despite their limitations, pragmatic research can provide valuable data for making decisions within the context of healthcare.

Results

Conclusions

They have populations of patients which are more closely resembling the patients who receive routine care, they use comparators which exist in routine practice (e.g., existing drugs), and they depend on the self-reporting of participants about outcomes. This approach has the potential to overcome the limitations of observational research, such as the biases associated with reliance on volunteers, and the limited accessibility and coding flexibility in national registry systems.
Pragmatic trials have other advantages, like the ability to leverage existing data sources and a greater probability of detecting meaningful differences than traditional trials. However, they may still have limitations that undermine their reliability and generalizability. These characteristics, according to the authors, can make pragmatic trials more useful and applicable in everyday practice. However they do not guarantee that a trial will be free of bias. The pragmatism principle is not a definite characteristic and a test that doesn't have all the characteristics of an explanatory study may still yield reliable and beneficial results.
